Understanding Scalp Skin Cancers
Cancers
of the scalp — including both melanoma and non-melanoma types — are increasing
in frequency, largely due to prolonged sun exposure and cumulative actinic
damage. As outdoor activity rises, so does ultraviolet-induced skin malignancy.
Surgical
excision remains the gold standard treatment, traditionally followed by
reconstruction using local rotation flaps. However, flap reconstruction can be
extensive and invasive.
πͺ A Simpler Surgical Approach
Dr.
Manstein presents over 30 years of clinical experience using two
straightforward reconstructive techniques:
1️⃣ Healing by Secondary Intention
- Ideal for defects smaller
than 4 cm
- Minimal long-term
disfigurement
- Avoids complex flap
procedures
2️⃣ Thin Full-Thickness Skin Graft
- Suitable for defects up to 8
cm
- Acceptable cosmetic outcome
- Can be performed under local
anesthesia
- Outpatient-based procedure
π Clinical Experience & Outcomes
Over
three decades, Dr. Manstein has treated 58 patients using non-flap
reconstruction — even in cases with possible periosteal involvement.
With
appropriate patient selection and careful pre-operative counseling, these
techniques have proven to be:

Dr. Carl
H Manstein is a board-certified plastic surgeon, certified by both the
American Board of Surgery and the American Board of Plastic Surgery. He
completed his plastic surgery residency at Duke University and serves on the
voluntary teaching staff at Temple University in the Department of Plastic
Surgery.
He has
also participated in international humanitarian missions, including work with Operation
Smile.
